Questionnaire pages are added via the Page Maintenance screen. To display the Page Maintenance screen, click Questionnaire Pages in the top left-hand corner of the Questionnaire Maintenance module screen.
Note that if you have another screen open on the desktop, such as the Page Entry screen, you may need to minimise or close that screen before you will be able to see the Page Maintenance screen. To minimise or close a screen, click or in the top right-hand corner of the screen respectively.
To add a new questionnaire page, follow the procedure below.

In the left-hand pane of the Page Maintenance screen where the category hierarchy is displayed, click |

Under the expanded branch of the category hierarchy, click the name of the category to which the new page is to be linked. For example, click Office if the new page is to be linked to the Office category. |

In the Display Code field, type a unique code that identifies the page and/or the sequence in which the page will be displayed within the page category. For example, if there are to be multiple pages created at the same level of the category hierarchy, the display code can be used to identify the sequence in which the page will be displayed (for example: 01). |

In the Name field, type a name that uniquely identifies the page (for example: Melbourne Office). This name will be displayed in the category hierarchy and will be prefixed by the code specified in the Display Code field. |

In the Page Heading field, type the text that is to appear at the top of the questionnaire page. |

If the page is to be included in the questionnaire workflow mandatorily whenever the page is applicable to a job order, click Is Enforced field so that it displays a tick. |

In the External Reference field, select at least one external reference to which the page will be linked. |

In the Page Category List field, select each page filtering category to which the page is to linked by clicking the relevant category names. |

Add the relevant questions, question groups and free text to the page. |

Once you have completed the procedure above, you can preview the appearance of the page by clicking. If you have finished working with the Page Entry screen you can close it by clicking near the top right-hand corner of the screen.
