To add a question to a questionnaire page, follow the procedure below. Note that the procedure assumes that a new page is open in the Page Entry screen.

If you want to add the new question to a question group that has already been added to the questionnaire page, click on the question group within the left-hand pane of the Page Entry screen so that the group is selected.

Otherwise, ensure that no items are selected (highlighted in blue) in the left-hand pane of the screen by clicking the mouse anywhere within the right-hand pane of screen.

In the left-hand pane of the Page Entry screen, click the Add dropdown action.

An item labelled New Item is added to the list of page components shown in the left-hand pane of the Page Entry screen, under the selected question group, or at the bottom of the list if no question group was selected. The right-hand pane of the Page Entry screen displays options for defining the attributes of a new question.

If the new question is to be a mandatory question that has to be answered before the questionnaire can be submitted, click the Mandatory field so that it displays a tick.
Skip this step if the question is not be mandatory.

In the Candidate Question field, type the text of the question as it is to appear on the questionnaire page.

The New Item tag in the left-hand pane of the Page Entry screen updates to display the text you typed within the Candidate Question field instead.

If the new question is to be mapped to a database field, click Mapped Question so that a tick appears next to the field.

Skip this step if the new question will not be a mapped question.

In the Answer Type field, select the answer type for the new question. Alternatively, if you enabled the Is Mapped Question option, select the database field to which the question is to be mapped.

An Options field grouping appears at the bottom of the screen displaying different fields depending on the answer type or database field selected.

In the Options field grouping at the bottom of the screen, set the available options as required for the question. The options available will depend on the answer type/database field selected in the Answer Type field.

Using the and buttons in the left-hand pane of the Page Entry screen, move the question up or down within the order of items that appear on the questionnaire page if necessary.

Click Is Knock Out, Is Scoring and/or Is Branching if the new question is to be a knock out, scoring and/or branching question respectively.

These options are available only if the answer type of database field selected in the Answer Type field supports knock out actions, scoring and/or branching and, if available, these options appear beneath the Answer Type field.

10.

If you enabled the Is Knock Out, Is Scoring and/or Is Branching options in the previous step, set the knock out, scoring and/or branching options as required.

11.

Click Save. A

confirmation message opens, indicating that the page has been saved successfullyThe item is saved.

Once you have completed the procedure above, you can preview the appearance of the page by clicking Preview. If you have finished working with the Page Entry screen you can close it by clicking Cancel. near the top right-hand corner of the screen.
