Questionnaire pages are added via the Page Maintenance screen. To display the Page Maintenance screen, click Questionnaire Pages in the top left-hand corner of the Questionnaire Maintenance module screen.
Note that if you have another screen open on the desktop, such as the Page Entry screen, you may need to minimise or close that screen before you will be able to see the Page Maintenance screen. To minimise or close a screen, click or in the top right-hand corner of the screen respectively.
To add a new questionnaire page, follow the procedure below.

5. | In the Display Code field, type a unique code that identifies the page and/or the sequence in which the page will be displayed within the page category. For example, if there are to be multiple pages created at the same level of the category hierarchy, the display code can be used to identify the sequence in which the page will be displayed (for example: 01). Alternatively, leave this field blank and the system will generate a display code automatically. This will be in the format Page <page number>, where <page number> will be the sequence number in which the page is created. Note that the display code will be shown as a prefix to the page name (for more information, see below) within the category hierarchy. | |
6. | In the Name field, type a name that uniquely identifies the page (for example: Melbourne Office). This name will be displayed in the category hierarchy and will be prefixed by the code specified in the Display Code field. | |
7. | In the Page Heading field, type the text that is to appear at the top of the questionnaire page. | |
8. | If the page is to be included in the questionnaire workflow mandatorily whenever the page is applicable to a job order, click Is Enforced field so that it displays a tick. | |

Available For field, use the Add action to select at least one external reference to which the page will be linked. Skip this step if you are creating a page linked to the Country level of the category hierarchy. | ||
10. | In the Page Category List field, select each page filtering category to which the page is to linked by clicking the relevant category names. A tick appears next the categories that you click. | |
11. | Add the relevant questions, question groups and free text to the page. Note that you cannot save a new page until the page contains at least one question, one free text item or a question group with at least one question. | |
